  1. Landlords who received $600 or more for machinery, pasture, farmland, or office space rent must indicate the amount in Box 1.
  2. Report any royalties of more than $10 in Box 2. Royalties cover license fees for copyrights, patents, and trademarks.
  3. In Box 3, report income of $600 or more that does not fit any other box on the form. This includes payments to participants in medical research studies, amounts paid to former self-employed insurance agents, monetary awards or prizes, and payments for taxable damages.
  4. In Box 4, enter the amount of withheld taxes for a person subject to backup withholding or who has not furnished a TIN.
  5. Box 5 is used to indicate fishing boat proceeds.
  6. Include medical and healthcare payments in Box 6.
  7. In Box 7, check the box if you sold goods of $5,000 or more to a person for further resale.
